Controlled Substance Licence for VTAs (issued by EPA - Environmental Protection Authority - previously ERMA)

Applicants need to be:
1.       17 years of age or over
2.       Adequately trained to handle that substance (have an approved handler certificate)
3.       Require the substance for their work
4.       Complete a fit and proper person assessment. 

Applicants will need to complete
1.       EPA application form (see link below)
2.       Approved Handler Assessment Paper
3.       Toxin Assessment Papers
4.       Work Assessment / Verification - completed statutory declaration form which has been witnessed by an approved person  (e.g. a JP) or satisfy a Test Certifier that you meet the work need requirements.

From 1 July 2011 EPA will no longer accept a firearms licence as proof that the fit and proper person criteria for a CSL has been met. All applicants will need to undergo a police check.    
This requirement from EPA is also why the fees have been increased - everyone now has to pay for a police check.

New Controlled Substance Licence: $390.00 inc GST
 
Renewal of Controlled Substance Licence: $255.00 inc GST

NB: The same form is used for a new application and the renewal of an existing licence.  When sending in a renewal application a signed declaration form also needs to be included.
